I don't see the word "all" being implicit in the term. The reason we're using "toxic masculinity" is consciously to point to iterations of masculinity that are indeed palpably toxic. It's like with the term "systemic racism" -- not all forms/practices of racism are inherently systemic, but the prevalent ones that ARE systemic need special attention directed at them, to combat them.
"Healthy masculinity" (or my preferred term, "nutric masculinity") are good antonym terms to juxtapose against "toxic masculinity."
